Once a year from now on (for life!).
Measure the child/adolescent/teen/young adult. Lots of questions about eating, bowels, sleep, school, social life, and family life. Parents/guardians will complete yearly screeners. Starting at 11 years old, the patient will complete their own screeners.
There are generally vaccines at the 11, 16, 17, and 18 year visits. Sometimes in between for the yearly flu shot, yearly covid shot, and the HPV vaccine series (to prevent cervical, anal, rectal, throat, and tongue cancers and genital warts).
These visits qualify for a sports physical, so bring any forms you need completed to the visit.
Schedule the appointment for your child's easiest time of day. Try not to schedule the appointment at your child's naptime. Tired Little (and big) people can get sad!
Try to feed your child breakfast, lunch, or a snack before the visit. Hungry little (and big) people can get grumpy!
Bring drinks and snacks in spill proof containers (if possible).
Bring comfort object like a favorite stuffed animal, blanket, or toy.
Bring books or toys for entertainment.
Talk to your child before the visit about what to expect. Always tell your child if there are vaccines and be truthful. This builds trust and respect, because your child knows they are safe and won't be surprised by you, the medical assistant, or the doctor. Teach them that vaccines are important and something we do to help them grow up big, strong, and healthy!
Bring a reward or plan for a reward after the visit to give your child something to look forward to. All people, little or big, need encouragement (like getting a paycheck) to do hard things!
Don't make promises you can't keep. Sometimes there are surprise tests (think throat swabs!), vaccines, or other owies (tests for anemia) that you don't expect. Please don't promise there won't be any pokes, vaccines, or owies. DO promise that you will prepare them the best you can and will always be there to keep them safe.
Here is a simplified list of vaccines that will be given at each well check. The schedule will be adapted if needed for illness, missed doses, or parental preference. For the full list and schedule click here: CDC Vaccine schedule Birth through 18 years .
This schedule lists the brand names of vaccines for curious parents. We use combo vaccines that cover multiple infections in 1 poke to minimize the number of pokes your child receives. Vaccines are available individually if needed for catch up or parent preference.

Usually your yearly well check will allow us to complete any sports forms you need. If you have had a well check in the last year, send in your form by the portal or drop it off at the office, and we will complete it within 48 hours.
If your program requires a specific date on the form, you may need to come in for a quick visit to update your exam.
If you have not had a well check in the last year, schedule your well check and bring in your physical forms to complete at the same time.
It's always easiest to complete the forms at the same time as your well check, so let us know if you need any forms completed while you're at that visit!

We can fully diagnose and treat ADHD, anxiety, and depression.
We will start the evaluation and coordinate with specialists for full diagnosis and treatment of developmental delays, Autism, trauma, and eating disorders. These require a team approach by specialists to make sure your child gets the best care.
We provide information, advice, and prescriptions or referrals for all types of hormonal and nonhormonal forms of contraception. We do not place implants or IUDs, and we can refer to safe providers who do.
We also diagnose and manage long, heavy, painful, irregular, and frequent periods.
We are gender affirming and will provide these services to all genders and all ages, up to 21 years.
We also do testing and treatment for sexually transmitted infections (STIs) without judgement. We are able to prescribe and manage PrEP for patients as desired.
Oregon law allows people of any age to access these reproductive health services without parental consent and with full privacy. The American Academy of Pediatrics encourages pediatricians to offer these services while educating patients and encouraging open communication and support from their parents.
We provide EASY access for medical care to teenagers!
At 15 years old teens can consent for all medical care without parent permission. This allows them to be seen in the office when their parents are unable to make it in. They can stop by before or after school for a well check, sports physical, vaccines, and for any question or concern.
At 14 years old teens can consent to medical care for mental health including therapy, counseling, and medications. This can be started without parent involvement, and before finishing treatment the parents must be involved unless it is unsafe for the teenager.
We will provide full privacy for teens while encouraging open communication with their parents. If at any point we are concerned about a teen's well being, we will make sure they have access to all available supports. If there is serious or imminent threat of suicide, we will inform parents/guardians. We will report all concerns of abuse or intent to cause harm as required by Oregon law.
